Essential Job Functions:

  • Provide all services and functions within Preferred Family Healthcare's Code of

Conduct; adhere to all company policies for employment.

  • Demonstrate understanding of health service systems and assist program

participants in identifying, securing, and sustaining a range of resources

needed to facilitate independent healthy living.

  • Coordinate care and participate with the clinician in the development of the client/patient's treatment plan based on individualized strengths, needs, abilities, and preferences of the person served by conducting case management assessments.

  • Consult and cooperate with community resources to facilitate linkage, referral, crisis management, advocacy, and follow-up with the focus on attaining goals as identified in the treatment plan.

  • Provide direct services to adults, children, and families; coordinates family involvement in the service-delivery process.

  • Help clients/patients navigate the system of financial entitlements to ensure they have adequate income and access to healthcare coverage.

  • Assist clients/patients with learning skills for successful personal health management and self-advocacy.

  • Monitor and assess consumer's functioning and progress; update team members with information relevant to supporting the treatment plan goals.

  • Demonstrate understanding of the overall nature, course, and treatment of mental illness.

  • Evaluate services that are provided to ensure that necessary improvements are made to increase outcomes.

  • Provide the continuity of contact for the client/patient throughout the entire service process.

  • Coordinate access to housing, social services, substance abuse treatment, and other community resources, and consult with these resources to assist clients/patients in the achievement of their goals.

  • Understand and respond to client/patient rights and advocate on their behalf.

  • Understand vocational rehabilitation services in a manner that facilitates access for clients/patients.

  • Develop support(s) for clients/patients who are employed or involved in employment programs; teach consumers how to maintain employment through solving problems that arise on the job.

  • Assist clients/patients in daily and independent living skills.

  • Assist clients/patients in learning appropriate social skills and behaviors.

  • Monitor and assess client/patient's behavior and functioning and provide therapists with information relevant to supporting treatment plan development.

  • Serve as a conduit and link between clients/patients and community services and resources.

  • All other duties and responsibilities, as assigned

Knowledge, Skills, and Abilities:

  • Knowledge of case management and rehabilitation methods, principles, and techniques, as they relate to mental and behavioral health.

  • Knowledge of the problems encountered by persons with mental illness and how to access the various community resources available.

  • Knowledge of the methods and modalities, as well as the legal requirements and court procedures related to treating mental illnesses.

  • Some knowledge of supervisory and training techniques and practices used in a mental/behavioral health setting.

  • Capable of communicating ideas clearly and effectively, both verbally and in writing to produce clear and concise reports.


Experience and Education Qualifications:

  • Bachelor's degree in social service, education, behavioral health, or other health-related fields from an accredited college is required.

  • Documented experience in the area of case management services is preferred.

  • Staff must demonstrate current certification as a Certified Behavioral Health Case

Manager (BHCM) II with the OK Dept. of Mental Health and Substance Abuse

Services.

Supervisory Requirements:

  • None

Employment Requirements:

  • Successful completion of a background check including criminal record, driving record, and abuse/neglect.

  • Completion of New Hire Orientation at the beginning of employment.

  • All training requirements including Relias Learning at the beginning of employment and annually thereafter.

  • A valid driver's license and an acceptable driving record are required.
