  • Be part of a CCBHC team with a focus on clients discharging from FCS' Crisis Care Center (CCC) C2- Urgent Recovery Center and transitioning to outpatient care.
  • Serve adult clients, age 18 and older, that are diagnosed with a severe mental illness, substance use disorder, or co-occurring disorder.
  • Embedded at a community based structured crisis center completing outpatient intakes and initial care plans to assist with clients transitioning from CCC C2 to outpatient services.
  • Provide a welcoming environment to clients who are seeking services through Family & Children's Services.
  • Introduce clients to the array of services provided by Family & Children's Services and assist them in obtaining services that suit that individual's recovery needs.
  • Meet clients where they are in terms of current mental health symptoms and treatment preferences.
  • Gather and complete all CCBHC data metrics and SAMHSA grant required National Outcome Measures (NOMS) data as needed.
  • Motivate clients to become and remain engaged and progress in treatment.
  • Gather client information necessary for screening, assessment, diagnosing, and intake process.
  • Link clients to primary medical care providers including embedded Morton Comprehensive Health Care clinic.
  • Assist clients complete Medicaid enrollment applications.
  • Coordinate with Crisis Care Center Discharge Planners to help client successfully link to outpatient services.
  • Organize the timing and application of services to facilitate client success.
  • Coordinate treatment with family, treatment advocate, and/ or significant other.
  • Provide education for clients and their families on recovery and prevention to empower them toward full participation in directing their own care or assisting family members as desired to assist with planning and directing care.
  • Promote access on demand to intervene when a client is at risk for harm to self or other, hospitalization, or involvement with law enforcement.
  • Promote independence and develop external support structures to facilitate sustained community integration.
  • Inform clients about community and natural supports and how to utilize these in the recovery process.
  • Assist clients in developing empowerment skills through self-advocacy.
  • Provide treatment planning and therapy (individual, group, couple, family) services for client on
  • Obtain consent for treatment from all responsible persons
  • Establish appropriate and productive therapeutic alliances with the client(s) and caretakers, as applicable.
  • Provide crisis intervention as needed
  • Complete safety assessments to include risk of violence towards self and others. Complete safety plans and/or psychiatric advanced directives with clients

A Master's degree in Social Work or its equivalent from an accredited school. Knowledge and experience in individual, group and family therapy. Knowledge and experience in conducting mental health assessment, diagnosis and treatment planning. Must demonstrate a clinical family systems orientation and ability to work with diverse populations with multiplicity of problems. It is required that all Therapists who are eligible for licensing by the State of Oklahoma be licensed annually and all other Therapists seek appropriate supervision and meet standards that make them eligible for licensure. Must possess a valid Oklahoma Driver License and satisfactory driving record and use personal automobile to travel to locations other than primary office. Ability to speak and write fluently in Spanish is preferred.

This job is classified as a "safety-sensitive" position as defined by the Oklahoma Medical Marijuana and Patient Protection Act. Due to the "safety-sensitive" classification, an employee in this position would be subject to drug and alcohol testing, including random testing. Marijuana is one of the substances included in the drug panel screening. Possession of a medical marijuana license will not excuse you from the testing process or the consequences of testing positive for marijuana per the Family & Children's Services Drug Free Workplace Policy, including possible revocation of a job offer or dismissal from employment.
