Team Supervisor oversees staff that are part of an inter-disciplinary CCBHC team that provide care to consumers with a Severe Mental Illness, primarily Depression, Anxiety, and PTSD. Team Supervisors implement with fidelity all CCBHC requirements, core services, and activities, including care coordination to integrate mental health, addiction, and physical health needs.
Team serves adult clients, age 18 and older, that are diagnosed with a severe mental illness, substance use disorder or co-occurring disorder. Team provides a combination of office and/or field-based services
