Position Summary:
The Pharmacy Specialist provides high-quality, cost-effective pharmaceutical care to specific patient populations pertinent to the assigned scope of practice.
Adheres to National Patient Safety Goals as appropriate based on the level of patient contact this position requires. Comprehensive Medication Management (CMM) Only: This Pharmacist provides comprehensive medication management (CMM) for patients referred by INTEGRIS Medical Group primary care physicians and employees and their dependents referred from INTEGRIS Health Partners care coordinators. ,- Graduation of a program in Pharmacy from an accredited school of pharmacy
- Current Oklahoma Pharmacy license
- Advanced degree in pharmacy and/or a specialized residency in drug information or equivalent experience
- 15 hours of approved continued education annually and the presence of an active license will be deemed proof of such training
- Specialized residency in drug information or equivalent experience
- Must be able to communicate effectively in English Comprehensive Medication Management Only:
- Board Certified Ambulatory Care Pharmacist or Board Certified Pharmacotherapy Specialist required within one year of hire.
- Post graduate year one (PGY!) residency or equivalent experience required
