Departmental Overview

This department is where successful use of data and evidence based nursing care meets quality improvement, and practice transformation. Our department includes Clinic Care Coordinators who work directly with Norman Regional Clinics and their patients to provide an enhanced level of health promotion, care coordination, and disease management. We enjoy a collaborative process with both clinic based staff, acute care staff, and post-acute providers to ensure continuity of care, so our patients can truly consider Norman Regional Health System their medical home. The Care Coordination Health Partner see's patients in the clinic and/or via phone conference to assist patients with health care navigation needs.

Job Summary

The Care Coordination Health Partner (CCHP) is responsible for assisting in delivery of services that address the Social Drivers of Health, referral assistance, and bridging gap between communities, individuals and providers. The CCHP will serve as an extension of the health care team and be responsible for helping patients and their families navigate medical services and community resources to improve population health and increase patient self-sufficiency. Works closely with the Care Coordinators to meet the patient needs towards set goals including hospital follow up for inpatient/ER visits, medication compliance, coordination of services, community resources, and addressing Social Drivers of Health. Supports the Care Coordination departments in coordinating, procuring and managing resources in order to facilitate quality care, improve patient cost outcomes and assist in assuring a smooth transition home or to the next level of care.

Primary Care & Pediatrics- 700 S. Telephone Rd, Moore

Primary Care NW Executive & NW Norman- 3201 W. Tecumseh Rd, Norman

Primary Care West Norman & West Moore- 3400 W. Tecumseh Rd, Norman


Qualifications

Education

  • Bachelors degree in any field.
  • In lieu of degree, will accept 4 years experience in either customer service, insurance, social work, or medical fields.

(Above requirements can be met by the equivalent combination of experience)

Experience

  • Two years customer service experience

OR

  • Two years experience in insurance field/social work/medical fields.
  • Two years experience in clinical setting preferred

Job Skills

  • Customer Service experience preferred
  • Bilingual preferred but not required.
  • Basic computer skills.
  • Excellent communication both oral and written.
  • Highly organized and self motivated

Compensation/Benefits

  • $18.08-$29.53/hr depending on previous work experience.
